If you're trying to renew your license, please use our online renewal system. Renewals take place from January 1 to May 31.
Forms for individuals
Apprentice exam application. Use this form if you are applying to become an apprentice by taking the board's written and practical exams. If you recently graduated from a North Carolina barber school, you may want to coordinate with your school to apply for the exam.
Registered exam application
. Use this form if you're have a current apprentice-barber license, have been an apprentice barber for at least one year, and want to take the board's practical exam.
. If you are a barber in another state and you want to receive a license in North Carolina, please download this form. Please note that not all out-of-state applicants should use this form, but the form has a series of questions to see if you qualify.
Note for military members, veterans, and military spouses:if you are a member of the military, a veteran, or the spouse of a member of the military, you may be eligible for endorsement without meeting all of the requirements described in this application. Please see our FAQ page with information about the different requirements for you, and please feel free to contact the board staff with questions.
Restore registered license. Use this form if your registered-barber license has been lapsed for more than four years and you want to reinstate the license.
Change of manager. If you have a change in mananger, you must file this form to notify the board. There is no fee for this form.
Pre-inspection of a barber shop (not required; use the barber shop permit application above if you are opening a shop). Use this form if you want to have an inspection of your shop before the official inspection. Individuals sometimes want a pre-inspection to make sure that the shop meets board regulations without delaying the shop opening date. This pre-inspection is not required and is only offered as a convenience for shops.
